How to create an NDA form

Creating a Non-Disclosure Agreement (NDA) form is a crucial step in safeguarding your business's confidential information.

Whether you're working with employees, contractors, or partners, an NDA ensures that sensitive data stays protected and proprietary knowledge remains secure.

For a successful NDA form, you'll need the following:

  • Appropriate fields, including email verification;

  • Option to sign it;

  • Option to export the results in PDF;

  • Secure system for storing the data.

🧩 Template: You can use the pre-made template and adjust it according to your needs. Starting from only adding your company's name to fully changing it.

Step 1: Create a form

Go to your dashboard, click on + New Formaloo, and choose Form. Then name the document.

This will open a pre-made dashboard with a template of the form inside it. You can customize both - the dashboard and the template too.

Let's start with the form.

To go to the form editor, click on the edit form.

Step 2: Add relevant fields to the form

You can go ahead and remove fields in the template and add relevant ones.

For the NDA form, you might need the following fields:

  • Text field

    • To ask for the other party's name

    • To type in information about parties disclosing information

  • Content field

    • To write terms and conditions or provisions of the document'

    • To write filler sentences, such as "and is by and between:"

  • Email verification field

    • To verify the other party's email

    • To get an electronic signature

  • Date field

    • To date the document

  • Multiple choice field

    • For the other party to agree to the form by checking the mark

  • Phone field

    • To collect response's side phone number

  • City field

    • To collect the other party's location

Tip: Don't forget to customize the text on the ending page.

Step 3: Customize the form

Add a logo, and cover picture, and change the form colors to match your company's branding. This all can be done from the design tab on the right sidebar.

Tip: Feel free to edit the texts by selecting them. The editing window will appear.

Step 4: Share the form

Once you're happy with the form, it's time to share it. Click on the share button in the form editor.

From here, you can change the slug (ending of the URL) of the form and also set your custom domain. Simply click on the ending and type in the desired words.

Make sure, they're all lower-case letters, and instead of space, use "-".

From here you can just copy the link and share it to get responses, or you can also embed it.

Step 5: Manage responses & export into PDF

There are different ways to view form responses. From our different data-view options, the one that might be preferred is a table view.

Important: Make sure to allow edit mode from the top right of the window to be able to customize the dashboard and expand the results.

To download responses as PDF, expand the desired answer, then click on Download PDF.
