The survey allows employees to provide feedback on their performance, fostering communication and understanding between the employer and the employee.

In this article, we will take you step-by-step to create a top-notch employee self-evaluation survey.

The employee performance evaluation form can be used as a document to evaluate an employee's performance during a predetermined period.

Managers and supervisors can use the form as a feedback tool to learn more about how their team members are doing and to pinpoint areas where they can improve.

Step 1: Starting from the blank form

To start, go to your dashboard, and make your survey from scratch by clicking “New Formaloo”.

There, you have options to create your survey and forms.

Choose 'Form' or 'Survey', whether you want to create a form or a survey. FormLoo offers various options for creating different types of forms.

If you choose to create a survey, FormLoo will automatically generate a default survey with a results page and charts. You can customize this survey later.

Once the form or survey is created, you can click on 'Edit base' to open the default template.

Here, you can edit the questions, add new ones, or customize the design according to your preferences.

After this, your form is ready to customize and will open up. You can already start adding the content.

By clicking on the "+ Add field" you can choose from our variety of questions and other features used to make your survey engaging and informative.

Step 2: Add the content and fields

To start adding content to your survey, you need to click on "+ add field" - this will open up all the features you can use, including date & time fields, signatures, and a variety of question types. Here's what to add and why you might want to add them.

Single/Multi-choice field

Pick this for simple questions with set answer choices, making responses easy to analyze and compare.

Long text field

Use this to get detailed and personal responses from survey-takers.

💡 Here are some question suggestions:

- What are three of your key strengths that have contributed to your success in your current role?

- Which areas of your performance do you feel you could improve? What specific steps would you take to address these areas?

- What are your professional development goals for the upcoming year? What concrete steps will you take to achieve these goals?

Score 0-10 field

Use this when you want number ratings, like for satisfaction or performance. It gives a clear measure for evaluation.

You can customize the maximum score according to questions.

Yes/No Field

This field allows you to get specific insights quickly and easily.

CSAT field

This measures satisfaction in a user-friendly way. You can use preferred emojis for a better display.

Like/Dislike field

Choose this question type for insights without requiring detailed explanations.

💡 Here are some question ideas:

- Do you like the company's culture and values?

- Do you like the work-life balance at the company?

- Do you like the opportunities for training and development at the company?

Dropdown field

This is useful when you want respondents to choose from a set list, ensuring consistency and providing an organized way for respondents to select.

Step 3: Adjust design & colors 🖌

Customizing the survey design to align with the organization's brand, incorporating logos and colors, is crucial for upholding a unified and professional image.
