How to add a title or description to each page or section of your form

Learn how to add page or section titles, descriptions, and images to multi-page forms using Classic layout and Content or Section Divider fields.

When you build a multi-page form, it’s often helpful to label each section clearly so users know what to expect. You may want to:

  • add a page title such as “Personal details” or “Payment information”

  • include instructions or a short description

  • insert an image (e.g., a visual guide, illustration, or example)

  • highlight important information before the questions on that page

This guide explains how to add titles, descriptions, or images to any page of your multi-page form, which field types to use, and what to keep in mind depending on your form layout.


1. One question at a time vs Classic form layout

In One question at a time layout, each page can show only one field.


That means you cannot add extra fields (like Content or Divider) on the same screen as your question.

If you want to:

  • show section titles or descriptions, or

  • place images above a group of questions

you’ll need to switch to the Classic form layout:


2. Switch to Classic layout and use pagination if needed

  1. Open your form

  2. Go to Design settings → set Form type to Classic

If you still want your form split into steps/sections:

  1. Click +Add field

  2. Add New page

  3. Use drag-and-drop to move existing fields onto the desired page, or add new fields right inside the page

  4. Repeat for each section you want

Each page can now include multiple fields, titles, and descriptions.


3. Use Content or Section Divider fields

You can use either Content or Section Divider field to display section/page titles and descriptions.

For both field types:

  • Title is required

  • You can optionally add a description, which supports:

    • Formatted text (bold, italics, etc.)

    • Links, Lists, Tables

    • Images (via /image in the field's description)

So in both cases you can create a rich header for your page or section.


4. When to use a Content field

Use a Content field when you want a flexible block at the top of a page or above a question, for example:

  • “Step 2 – Payment details”

  • A short paragraph explaining what’s coming

  • An image that visually illustrates the section

How to add it:

  1. Go to the page where you want the header

  2. Click +Add fieldContent

  3. Add a title (required)

  4. Add a description with text, links, lists, tables, and/or /image for visuals

  5. Drag it to the top of the page if needed


5. When to use a Section Divider field

A Divider field behaves almost the same as a Content field, with one extra visual element:

➡️ It always includes a horizontal divider line.

This is useful when you want to visually separate sections on the same page.

How to add it:

  1. Click +Add fieldSection Divider

  2. Add the title (required)

  3. Add an optional description (supports text, links, lists, tables, images)

Place it just above the fields that belong to that section.
