How to add images to your form

Learn all available ways to add images to your form, including cover images and per-page visuals, and how each option works in different layouts.

Formaloo supports several ways to add images to your forms. You can use a single form-wide cover image, or insert individual images on specific pages or next to specific questions. This guide explains all available options, how they behave, and which method fits best depending on your chosen form layout.


1. Add a Cover image (one image per form)

A cover image applies to the entire form, so there's no option to set an individual cover image on different pages of the same form.

You can set it directly in the Form Editor by clicking Upload Cover image:

In Design settings, under Layout, you can choose one of the 6 placement options for your form's Cover image:

  • Top

  • Center

  • Full left / Full right

  • Left / Right

How the Cover image displays on the form

  • Top or Center: the cover image appears only on the Welcome page, or at the top of the form if it’s a one-page form.

  • All side positions (left/right variants): the cover image displays the same way on every page of the form.

If you need individual visuals per page, use one of the options below instead of a Cover image.


2. Add images inside individual fields' descriptions

You can insert an image directly into any field’s description.

How to add:

  1. Click on the field's Description

  2. Type /image and upload an image file or paste an image URL

This is the best option when:

  • You want an image to show right next to a specific question/field

  • You’re using a one-question-at-a-time layout, so you cannot add additional fields on the same page

Images added this way appear right inside the field's description below its title:


3. Add images using Content fields

You can also add individual standalone images by inserting a Content field anywhere in your form.

To add:

  • +Add fieldContent field → type /image to add an image inside it

This allows you to place images:

  • Next to any separate field or a group of fields

  • On specific pages (e.g., at the top of each new quiz section)

  • As decorative or explanatory elements mid-form


4. Images in fields in Classic forms vs One-question-at-a-time forms

  • Classic form:

    • You can add multiple fields on a single page

    • Content fields and questions can appear together

    • You can also add pagination in a Classic form by clicking + Add field → New Page, and grouping multiple fields on separate pages

  • One question at a time:

    • Each field (including Content fields) appears on its own page

    • Two fields cannot appear on the same page

    • A Content field with an image will become a separate page

    • If you want an image to appear with the question on the same page, insert the image inside the field's description instead


5. Choosing the right method based on your layout

If you want one shared image across the whole form

✔ Use a cover image
✔ Choose a side placement to show it on all pages
✔ Choose Top/Center if you only want it on the welcome page/in the form header

If you want different images on each page

✘ Not possible via multiple cover images (cover image = one per form)
✔ Insert images via Content fields or field descriptions

If your form is One-question-at-a-time

✘ You cannot display multiple fields (e.g., content field + question) on one page
✔ Use field descriptions for images on the same page as the question
✔ Content fields will each appear on their own page


6. Summary of what’s possible

  • You can only have one cover image per form

  • Cover images cannot differ per page

  • For per-page or per-question images, use field descriptions or Content fields

  • In One question at a time, images will only appear on the same page with the question if they are added to the field's description (Content fields will occupy their own page in One question at a time)

  • Classic form offers full flexibility for mixing multiple fields on the same page while keeping the form divided into parts/sections
