One‑question‑at‑a‑time (1QAT) forms are designed to help respondents focus on a single question at each step. Adding individual cover images per question lets you visually reinforce that focus, add context, or set the tone as users move through your form.

Why use individual cover images?

Using a different cover image for each question can help you:

  • Create a more engaging, story‑like form experience

  • Add visual context to specific questions

  • Reduce cognitive load by visually separating steps

  • Match imagery to different topics or sections

This is especially useful for onboarding flows, surveys, quizzes, and application forms where each step has a distinct purpose.

💡 If you only need one static image for the entire form, a single default cover image may be enough. Individual cover images are best when each question benefits from its own visual context.


How individual cover images work in 1QAT forms

In one‑question‑at‑a‑time (1QAT) mode, each question is shown as its own step. When you upload a cover image for a question, that image is shown while the question is active.

If a question does not have its own cover image, Formaloo automatically falls back to the form‑level, default cover image (if one is set).

As respondents move to the next question, the displayed cover image updates dynamically to match that step – either the question‑level image or the form‑level fallback.


How to add individual cover images in a form

ℹ️ Individual cover images are currently supported only in one‑question‑at‑a‑time (1QAT) forms.

Adding individual cover images per question works with side layouts (Right, Left, Full right, Full left) where the cover image appears on the side next to each question.

With Top and Center layouts, the cover image only appears once at the beginning of the form.

Step 1: Choose how cover images appear in your form

Start by opening your form’s Design settings and selecting a 1QAT layout.

  • Top and center layouts display the cover image only on the first step of the form.

  • Side layouts display the cover image next to each question, on every step.

If you want cover images to appear alongside every question, make sure you’re using one of the side layouts:


Step 2: (Optional) Set a default cover image for the form

If you’re using a side layout, you can start by setting a default cover image in the form’s Design settings:

This image acts as a fallback and will be shown next to all questions unless a question has its own individual cover image.

ℹ️ Default cover image = fallback
Your form’s default cover image is shown for any question (or page) that doesn’t have an individual image – question‑level images don’t remove it but overwrite it.

If you add and then delete an individual cover image, the fallback default cover will take its place again.

ℹ️ Switching layouts
If you switch from a side layout (left/right) to a top or center layout, question‑level cover images won’t be shown – but they’re preserved and will appear again if you switch back to a side layout.


Step 3: Add individual cover images to questions

Once your layout and (optional) default cover image are set:

  1. Click on a question in your 1QAT form,

  2. Upload a cover image for that specific question,

  3. Repeat for any other questions where you want a custom image:

For questions without an individual image, the form’s default cover image (if set) will be used automatically.

The preview updates instantly, so you can see exactly how images change as users move through the form.
