Why use individual cover images?
Using a different cover image for each question can help you:

How individual cover images work in 1QAT forms
In one‑question‑at‑a‑time (1QAT) forms, each question is shown as its own step. When you upload a cover image for a question, that image is shown while the question is active.
If a question does not have its own cover image, Formaloo automatically falls back to the default cover image (if one is uploaded in your design theme).
As respondents move to the next question, the displayed cover image updates dynamically to match that step – either the question‑level image or the form‑level fallback.

How to add individual cover images in a form
⚠️ Individual cover images are only supported in 1QAT forms.
Adding individual cover images per question works with side layouts (Right, Left, Full right, Full left) where the cover image appears on the side next to each question.
With Top and Center layouts, the cover image only appears once at the beginning of the form, so it's not applicable.
Step 1: Choose how cover images appear in your form
Start by opening your form and navigating to Design. There you'll see the theme currently applied to your form.
If the theme already uses a 1QAT form type, you can proceed to the next steps,
If you'd like to switch from Classic form to 1QAT, you have a few options:
Edit the currently applied theme (changes will affect all forms using that theme)
Duplicate the theme and create a variation specifically for this form
Create a completely new theme
Once you're working with a theme configured as a 1QAT form, choose one of the supported layouts:
Top and center layouts display the cover image only on the first step of the form – so setting individual images per page is not applicable with this layout,
Side layouts display the cover image next to each question, on every step.
If you want cover images to appear alongside every question, make sure you’re using one of the side layouts:
Step 2: (Optional) Set a default cover image for the form
If you’re using a side layout, you can start by setting a default cover image in the form’s Design theme settings:
This image acts as a fallback and will be shown next to all questions unless a question has its own individual cover image.
ℹ️ Default cover image = fallback
Your design theme's default cover image is shown for all questions (or pages) that don’t have an individual image – individual, page-specific covers don’t remove it but overwrite it.
If you add and then delete an individual cover image, the fallback default cover will take its place again.
ℹ️ Switching layouts
If you switch from a side layout (left/right) to a top or center layout, question‑level cover images won’t be shown – but they’re preserved and will appear again if you switch back to a side layout.
Step 3: Add individual cover images to questions
Once your layout and default cover image are set:
Click on a question in your 1QAT form,
Upload a cover image for that specific question,
Repeat for any other questions where you want a custom image:
For questions without an individual image, the form’s default cover image (if set) will be used automatically.
The preview updates instantly, so you can see exactly how images change as users move through the form.

What happens if I change the layout?
If you switch to a layout that doesn’t support banners, existing question‑level images are preserved but not rendered.

No. Cover images automatically adapt to different screen sizes to maintain readability.
