How to add a title, logo, and description to your form

Make your form stand out by adding a custom title, logo, and description. You can also center your content or hide the default title.

When you enter your form title and upload a logo, they’ll appear at the top-left corner of your form by default.

Add a title, logo, and description

To add these:

  1. Open your form in Edit Mode.

  2. Click on the title of your form and change it.

  3. In the Description box, add text about your form or organization. You can use the editor to format your text, change colors, insert links, and even insert your logo inside the description instead of displaying it at the top.

  4. Add your logo by clicking on "Upload logo" on the top of your form title

🎨Customize the layout with design settings

You can further control how your content looks:

  • To center your logo and title: You can delete the default logo from the top-left and reinsert it into the Description box where you have full control over layout and alignment.

  • To hide the default form title:

    • Go to the Design tab.

    • On the right side, toggle off “Show Form Title”.

Set the title and favicon on custom domains

If you're using a custom domain, your form will automatically use the form title as the page title, and your uploaded logo as the favicon.
