How to add a contact info to your form

Add a complete contact info to any form in one click. First name, last name, email address, and phone number, laid out neatly in two rows.

When to use this

  • You're building a lead gen, registration, or booking form and need basic contact details fast

  • You want a clean, consistent layout without configuring each field one by one

  • You're replicating a standard contact block across multiple forms

How it works

When you add the Contact info shortcut, Formaloo automatically creates the following fields:

  • First name (text field)

  • Last name (text field)

  • Email address (email field)

  • Phone number (phone number field)

Default layout

All 4 fields are added with a 50% width and arranged in two rows:

  • First name and Last name

  • Email address and Phone number

This layout keeps your form structured and easy to fill out.


How to add it to your form

  1. Open your form in edit mode

  2. Click on Add field

  3. Select Contact info

The fields will be added instantly to your form.

💡 Each field works like any normal field. You can rename, reorder, or delete them independently. Need extra fields like company or job title? Add them separately from the field picker.

Once added, your contact section is ready to go. Edit any field, adjust the layout, or start collecting right away.
