To change the name and the image of the app/form/survey, go to the dashboard and click on the desired formaloo you'd like to customize.

From here, double-click on the name displayed in the upper left corner of the page. once you double-click, you can insert the name you'd like.

The same goes out for the image. Click on the icon displayed next to the name and choose the desired image.
