What's my formaloo's slug?

You can find the slug of your formaloo in the address bar of your form, when you click on the editor of it:
Just copy the slug before /edit



Question: My Zapier asks me about the SLUG, what should I do?

Just copy only the slug part of the URL and then paste it in the zapier section.
